Start a consultation
When you need this
A patient has been checked in and it's their turn with the doctor. Starting the consultation tells the front desk the patient is with the doctor, and opens the consultation note.
The app calls each visit an encounter. The check-in makes one for you.
Steps
From the day's list (the quickest way):
- Open Appointments in the menu. The Today tab shows everyone booked or checked in today. Times are the practice's own clock, even on a computer set to another time zone.
- Find the patient. A checked-in patient waiting to be seen has the status Waiting. If you're a doctor, press My patients to see only the patients checked in or booked for you.
- Press Start consultation on their row. On a phone, press ⋮ on the patient's card, then Start consultation.
From the encounter itself:
- Open Encounters in the menu and click the patient's row on the Active tab.
- The encounter's status is Not started, Waiting (for the nurse), With the nurse, or Waiting for doctor once the nurse has sent the patient to you. Press Start consultation at the top right. (While the patient is waiting for or with the nurse, it's the button beside the nurse's Start triage or Send to doctor.)
What you should see
The patient's encounter opens, its status changes to In progress (from any of those), and the Consultation note opens on the right, ready to write. See Write the consultation note.
Back on Today, the patient's status is With doctor and their button says Continue consultation. Press it to go back to the encounter at any time.
The encounter page shows, from the top:
- The patient's name, the status, and under it their gender, age and when the visit started.
- The Vitals card: the latest reading of each vital in this visit. See Record vitals.
- The tabs: Summary, Vitals, Consultation notes, Diagnoses, Medications, Lab Tests and Attachments. A number beside a tab is how many things it holds, for example Diagnoses (2).
- On Summary: the Consultation note, Diagnoses, Medications and Lab tests cards, and beside them Previous visits (with Patient record) and View activity log. Each previous visit shows its date, its status and, under them, what it was for: the chief complaint from its note, or its main diagnosis (marked Diagnosis) when no complaint was written. The doctor's name shows when another doctor saw the patient that time. Click the date to open that visit. If your role can see invoices, a Billing card shows the visit's invoice and its status. On a medical aid visit the invoice reads Partly paid until the medical aid pays, so the card also shows Medical aid (what it pays, or No authorization / Not sent yet / Waiting for answer / Declined), Patient paid and Patient owes (Not known yet until the medical aid answers). With no invoice it says No invoice for this visit., or No charge for this visit. for a free one. If the visit was billed on an invoice made separately, Link an invoice on that card opens the check-in, where you pick the invoice. Under it, Visit times shows when the patient was Checked in, was With the nurse, was Sent to the doctor (with how long they were with the nurse), was With the doctor (the time you pressed Start consultation), and when the Visit ended, each with how long they waited before it (Waited 48 min, in orange when it was 30 minutes or more), then the Whole visit, or So far while the visit is open. The wait for the doctor counts from Sent to the doctor, or – if the nurse didn't press it – from the nurse's last record. A visit only the nurse saw ends with how long it was with the nurse. A step the visit skipped, such as the nurse, isn't listed. A visit nobody ended is ended automatically a day after check-in; its card then says Ended automatically, end time not recorded, with no Whole visit time. A cancelled visit shows Visit cancelled instead, also with no Whole visit time. Visits checked in before Visit times started don't have the card. If your role can open the report, Report opens Visit times.
If something goes wrong
- There's no Start consultation button: your role can't run consultations. Ask the person who manages staff logins. Otherwise:
- On Today, the button shows only for a patient who has been checked in, and not once their visit is finished or cancelled.
- On the encounter itself, it shows for any encounter that is Not started, Waiting, With the nurse or Waiting for doctor, checked in or not, and for one In progress with no note yet. Open the encounter from Encounters and start it there.
- There's no My patients button: your login isn't linked to your doctor record yet. Someone who can edit doctors opens Setup › Doctors, edits your record and picks you under Signs in as (see Add your doctors).
- A patient is missing under My patients: the front desk didn't pick you as the clinician at check-in, or picked another doctor. Press Everyone to see the whole day.
- The status says Waiting for doctor: the nurse has sent the patient to you. Press Start consultation as usual. If it says With the nurse, the nurse hasn't finished: you can still press Start consultation if you see the patient now.
- A message appears instead of the note: the consultation didn't start. Read the message, then try again. If it says the encounter can't be started, it may have been cancelled: check its status.
