Record vitals

When you need this

You're seeing the patient before the doctor (triage), or you've taken their blood pressure, temperature, weight or other readings during the consultation. Record them on the visit so the doctor sees them at the top of every tab, then send the patient to the doctor.

Steps

  1. Open the patient's encounter (from Encounters, or Continue consultation on Appointments › Today).

  2. While the patient is Waiting for you, press Start triage at the top right: the status changes to With the nurse. (Pressing Record vitals without it starts triage too.)

  3. On the Vitals card, press Record vitals. The Record vitals form opens on the right.

  4. Date and Time: when the readings were taken. They start at now.

  5. Blood pressure: the two numbers in their own boxes, for example 120 and 80.

  6. Fill in the readings you took: Heart rate, Temperature, SpO2, Respiratory rate, Blood glucose, Weight, Height. Each shows its unit beside the box. Leave out what you didn't take.

  7. With weight and height in, the form works out the BMI for you.

  8. Pain level: press a number from 0 (none) to 10 (worst).

  9. For a pregnant patient, turn on Pregnancy readings and fill in Gestational age, Height of fundus, Lie and Fetal heart rate. (The switch isn't offered for a male patient. Once a visit has pregnancy readings, the next reading opens with the switch already on.)

  10. Save:

    • Save and send to doctor when you're done with the patient: they go back to wait for the doctor.
    • Save when you still have more to do (an IUD, a dressing). When you're done, press Send to doctor at the top right.

Once you've sent the patient, or the doctor has them, the form has just Save vitals.

What you should see

The form closes and the Vitals card shows the new readings, with Latest and the time they were taken. Each vital shows its newest reading in this visit, so a heart rate taken later doesn't hide the blood pressure taken first. The visit's status – on the encounter, on Today, on the check-in and in the Encounters list – is With the nurse while you have the patient, and Waiting for doctor once you've sent them. Nothing you record moves the visit to the doctor: only the doctor's Start consultation does.

A visit that doesn't need the doctor (family planning, a dressing): when you're done, press Actions › End visit at the top right. The app asks End the visit?; press End visit. It's offered once you've recorded something on the visit.

To see every reading, press All readings on the card (or open the Vitals tab). Click a reading there to change it: the form opens as Edit vitals, and you save with Save changes.

If something goes wrong

  • "Enter at least one reading.": the form is empty. Fill in at least one vital.
  • "Enter both numbers, e.g. 120 / 80.": fill in both blood pressure boxes.
  • "The first number should be the higher one.": the blood pressure boxes are the wrong way round.
  • "Check the numbers: between 30 and 300 over 10 and 200." or "Check this: between … and …": the number looks like a typing slip, for example 368 for 36.8. Check it and type it again.
  • There's no Record vitals button: the visit is finished, or your role can't record vitals.
  • There's no Start triage or Send to doctor button: the patient is already past that step (sent, or with the doctor), or your role can't update encounters.
